Chief Executive Officer
Mike has more than 35 years of experience in the healthcare industry. Prior to becoming Chief Executive Officer for Evolution Health, he served as the CEO and President for Southern Care, Inc., a national hospice provider from 2009-2014. He served more than 30 years in the hospital industry in various executive capacities for national hospital companies National Medical Enterprise (now Tenet), HCA and Triad Hospitals.
His last hospital executive position was as Chief Operating Officer for Triad Hospitals from 1999-2007. He has served on several healthcare boards, including Triad Hospitals, The Federation of American Hospitals and Vanguard Health Systems. Mike received his Bachelor of Science degree in Accounting from the University of New Orleans and his Master of Business Administration from Pepperdine University.
Chief Operating Officer
Kurt serves as Chief Operating Officer for Evolution Health. Kurt has an extensive background leading home health and hospice organizations. Prior to coming to Evolution, he served as President of Allegheny Health Network's Home health and Hospice division. He held senior roles with Celtic Healthcare including Chief Operating Officer from 2005 until 2018. He also served as President of Celtic Living Assistance Services, a subsidiary.
Kurt is active in several professional associations and he has been published in trade periodicals. He has been invited faculty at professional conferences on numerous topics related to home health care. He earned his Bachelor of Science degree in Health Sciences and Master’s Degree in Physical Therapy from Duquesne University.
Chief Financial Officer
Mike Rzendzian (CPA, MBA) serves as the Chief Financial Officer of Evolution Health with responsibilities that include the overall financial management of the company, its financial reporting and transparency, and management of multiple corporate functions. These functions include Accounting, Information Systems, Payer Contracting, Revenue Cycle and long-range planning.
Mike has more than 25 years of experience and has previously held the role of Chief Financial Officer with multiple mid-size healthcare companies including Healthy Living at Home, Emerus Hospital Partners, Diabetes America and Nexus Health Systems – the latter where he was recognized by the Houston Business Journal as one of the top turnaround CFO’s in Houston, TX.
Vice President, Human Resources
Tom has 30 years of experience in Healthcare Human Resources. Prior to becoming the Vice President of Human Resources for Evolution Health, he served as the Group Vice President of Human Resources for CHRISTUS Health. During his career, Tom has served as the Senior Vice President of Human Resources/CHRO for Trinity Mother Frances Health System as well as Vice President of Human Resources/CHRO for the Seton Healthcare Family.
Tom received his Bachelor’s degree in Psychology from the University of Missouri – Columbia and his Master’s degree in Industrial/Organizational Psychology from Lamar University.
Vice President, Employee Engagement
Regina Caldwell serves as Vice President of Associate Relations for Evolution Health. Her organizational development and strategic initiatives contribute to the policy and procedure implementations. She started her career with the Envision family in 2003. During her tenure, she has worked with all business segments providing Human Resources support.
Regina has more than 10 years' experience integrating human resources with the overall business strategy and objectives. She brings expertise in HR policy design and administration, HRIS knowledge, recruiting and retention and mergers and acquisition.
Regina earned a Bachelor of Business Administration at Texas A&M University at Commerce. She then attended Amberton University in Dallas, Texas where she earned an MBA with a concentration in Human Resources.
Bobi Rose, RN
Senior Vice President, Ascension at Home
Bobi has made delivering and improving the quality of clinical care her life’s work. She has extensive experience as an executive leader for clinical and operational improvement in the home care setting. Prior to assuming her role as SVP for Ascension at Home, she held executive roles with several national home health providers. She has published in clinical journals and is recognized as a national expert in OASIS.
Bobi graduated with her Bachelor of Science in Nursing from Maryville University, her Diploma in Registered Nursing from the Baptist Memorial Hospital School of Nursing in Memphis, TN. Bobi is Certified in Wound Care, Catherization and holds a Certificate for OASIS Specialist- Clinical.
Jeanne Kalvaitis, RN, BSN
Senior Vice President of Hospice Operations
Jeanne Kalvaitis, RN, BSN serves as Senior Vice President of hospice operations for Evolution Health. Jeanne has dedicated her career to home health care and hospice from her early days as a nurse serving as the first contact with patients and caregivers referred for hospice to providing clinical leadership in various senior management roles over the past fifteen years. Throughout her career, Jeanne has focused on understanding the importance of the patient’s choice for treatment options and of ensuring a smooth care transition from hospital to home or hospice care. Before coming to Evolution, she served as Vice President of Hospice Clinical Services at Encompass Home Health. From 1990 until 2014, Jeanne worked for Vitas Healthcare Corporation, where she held various hands-on and senior leadership positions.
